This hybrid role combines direct resident care responsibilities with team leadership, staff support, training, and community relationship-building. The ideal candidate will have a strong commitment to enhancing our residents' quality of life by assisting with daily activities and ensuring their safety and well-being. In addition, this individual will serve as a positive ambassador for Truthville by participating in external meetings, building relationships with healthcare professionals and referral sources, and communicating the Truthville difference to prospective partners and clients. This position requires a blend of caregiving expertise, leadership ability, professionalism, communication skills, and emotional intelligence. Duties & Responsibilities Resident Care Responsibilities Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting Prepare nutritious, well-balanced meals and participate in meal planning and cooking to support residents' health and comfort Maintain a clean, safe, and comfortable living environment for residents by performing regular housekeeping, laundry, and cleaning tasks Provide safe and compassionate mobility support for residents, using proper lifting techniques and universal precautions to protect both residents and employees Follow the established daily schedule and each resident's individualized care plan, including participation in scheduled activities Engage residents in social activities to promote mental well-being and companionship Administer centrally stored medications as per care plans while ensuring compliance with Title 22 licensing regulations and guidelines, HIPAA, and guidelines for both resident and employee safety Monitor and document changes in residents' behavior or health in the care management system, and promptly report concerns to the Administrator Provide specialized care for individuals with Alzheimer's disease or other forms of dementia Facilitate clear and timely communication with healthcare professionals and residents' responsible parties to support effective, individualized care plans Monitor residents' activity levels, nutrition, functional abilities, and psychosocial well-being, taking timely action to support and promote overall health and quality of life Respond promptly to resident emergencies, following established protocols and applying first aid training to ensure resident safety and well-being Seek assistance or escalate concerns to the Administrator if assigned duties cannot be safely or fully completed Carry out additional assignments or responsibilities as directed by the Supervisor or Administrator to support resident care and facility operations Leadership & Team Support Responsibilities Serve as a lead caregiver and role model by promoting professionalism, teamwork, accountability, and compassionate resident-centered care Provide on-the-job training, coaching, and guidance to caregivers and new team members to support quality care delivery and regulatory compliance Reinforce Truthville policies, procedures, infection control practices, and resident care standards Support the Administrator in identifying performance concerns and documenting verbal and written warnings as part of the employee disciplinary process Assist with shift coordination, workflow support, and communication between caregivers and leadership to promote operational consistency Help ensure caregiver documentation, medication practices, and resident care activities are completed accurately and timely Company Liaison & Outreach Responsibilities Represent Truthville professionally during external shared meetings, networking opportunities, tours, and one-on-one meetings with healthcare and community stakeholders Build and maintain positive relationships with discharge planners, placement agents, nurses, social workers, case managers, hospice providers, home health agencies, and other referral partners Communicate Truthville's mission, values, services, and care philosophy to prospective clients and referral sources Help educate external stakeholders about Truthville's proactive, pulse-driven, connected, and conformable care approach Support occupancy growth efforts by helping generate and nurture referral opportunities Collaborate with leadership on outreach initiatives, follow-ups, and relationship-building activities within the community Experience & Qualifications Candidates should possess experience in caregiving or related fields such as: Assisted living, dementia care, home health, hospice care, or related senior care experience Minimum of two years of caregiving experience in assisted living homes, nursing homes, memory care communities, or similar care facilities Experience working with elderly individuals, including residents with dementia or cognitive impairment Prior leadership, mentoring, shift lead, training, or supervisory experience is preferred Comfortable providing coaching, feedback, and performance guidance to colleagues in a professional and respectful manner Strong verbal and written communication skills with the ability to communicate effectively with residents, families, healthcare professionals, and referral partners Professional presentation and interpersonal skills suitable for external meetings and community engagement Familiarity with resident observation techniques, direct support practices, and documentation systems Ability to exercise sound judgment, maintain confidentiality, and remain calm in high-pressure situations Ability to work independently while collaborating effectively within a team environment Must be at least 18 years old Criminal clearance required Tuberculosis (TB) clearance required First Aid and CPR certification required Medication training/certification preferred CNA certification preferred Valid driver's license and reliable
