Job Description

Scheduling Lead - InHome Services (IHS) SummitWest Care | Grand Junction, CO FullTime | OnSite | Monday-Friday $19.00-$24.00/hour (DOE) SummitWest Care is a community-based nonprofit home health agency serving Mesa County. Our In-Home Services (IHS) program provides essential support to individuals who need assistance to remain safely in their homes. Consistent, reliable scheduling is a key part of that care, and we are looking for a Scheduling Lead to take primary ownership of that function. This position serves as the lead coordinator for day-to-day scheduling operations and is responsible for managing caregiver coverage, responding to changes in real time, and keeping communication clear between clients, caregivers, and the internal team. The work is fast-moving and requires someone who can stay organized, make quick decisions, and keep things moving when plans change throughout the day.
What You'll Do:
SchedulingCoverage Coordination Coordinate caregiver schedules and client assignments based on care plans and availability Manage call-offs, schedule gaps, and other changes to maintain continuity of care Communicate schedule updates clearly and promptly with caregivers and clients Support building and maintaining consistent, reliable schedules CaregiverClient Support Serve as point of contact for scheduling needs, questions, and updates Build professional relationships with caregivers to support engagement and retention Work with clients and families to ensure care expectations are met and maintained Operations Coordination Balance multiple priorities and scheduling demands throughout the day Maintain accurate records, scheduling notes, and documentation Collaborate with intake, admin, HR, and leadership to support overall program operations General Office Support Answer and triage incoming calls Respond to client, caregiver, and family questions and concerns Adapt to shifting needs in a team-based environment Home health scheduling involves real people, real needs, and constant change. Calm communication, quick problem-solving, reliability, and compassion are essential in this role. What We're Looking For Strong communication and customer service skills Ability to stay organized while managing multiple priorities Calm, solution-focused approach to challenges and scheduling puzzles Team oriented mindset with willingness to step in and help Reliable, logical, detail-oriented, consistent follow-through Preferred but not required: Experience in home health scheduling, home care, healthcare, dispatch, or customer service roles Familiarity with Medicaid, IHS, Health First Colorado Familiarity with WellSky Personal Care (ClearCare Online) What We Offer Generous
