Orange, CT Job Details Full-time $55,000 - $65,000 a year 1 day ago Qualifications Achieving sales targets Full Job Description Constellation Health is seeking a dynamic Full-Time Home Care Liaison to join our team! We are seeking a motivated, outgoing, and detail-oriented professional to join our private duty home care team as a Home Care Liaison. This position is responsible for the external and internal sales of Constellation Private Duty. This role expects the Liaison to go into the community and build new referring accounts, expand on existing referring accounts and close clients by explaining the value our service has to offer with clarity, compassion and confidence. In addition to external and internal sales and family-facing responsibilities, this role provides backup support to office staff and participates in a shared on-call rotation to ensure continuity of care. Key Responsibilities Externally sell and build business relationships with nursing homes, assisted living facilities, independent living facilities, hospitals, doctors' offices, elder care attorneys, conservators, geriatric care managers, etc. Serve as the first point of contact for Accounts and families seeking private duty, non-medical home care Build trust quickly through warm, professional, and compassionate conversations Assess family needs and guide them toward appropriate services and next steps Confidently move conversations toward scheduling in-home consultations Accurately gather, document, and maintain client information and intake details Coordinate closely with scheduling and care management to support smooth service starts Represent the agency professionally to families, referral sources, and community partners Provide backup administrative and coordination support to office staff as needed Participate in a shared on-call rotation to assist with urgent after-hours needs Follow up consistently to ensure a high-quality family experience What We're Looking For Outgoing, energetic, and personable salesperson who is comfortable engaging with people in person or by phone Strong communicator with the ability to guide conversations resulting in closing the business Relationship-driven with a consultative, service-oriented mindset High attention to detail and strong follow-through Organized, dependable, and adaptable in a fast-paced environment Willing to support the broader team when needed Calm, responsive, and reliable during on-call responsibilities Motivated to grow within a mission-driven home care organization Preferred Experience (Not Required) Account or client-facing experience in sales, preferably in the healthcare environment Experience working with families, seniors, or community partners Phone-based or consultative sales experience Why Join Us Purpose-driven work supporting families during meaningful and often emotional moments Collaborative, team-oriented culture Lucrative bonus plan Training, mentorship, and ongoing support provided Opportunity to develop strong skills in care coordination, relationship management, and non-medical healthcare sales Clear pathways for professional growth Why Constellation?

Qualifications:
Bachelor's degree in Business, Marketing, Healthcare, or a related field preferred. Minimum of 2 years of experience in sales, preferably in non-medical home care, healthcare, or a related field. Proven track record of meeting or exceeding sales goals in a service-oriented environment. Strong interpersonal and communication skills, with the ability to build rapport quickly. Knowledge of non-medical home care services. Ability to work independently and manage time effectively to meet performance targets.
