Job Summary Home Sweet Home Care of SWFL is seeking a motivated, relationship-driven Home Care Liaison to expand our presence within the community and develop referral partnerships throughout the region. This position serves as the face of our agency and is responsible for building strong relationships with assisted living communities, independent living communities, hospitals, rehabilitation centers, physician offices, case managers, social workers, and other healthcare professionals. The ideal candidate is personable, professional, organized, and passionate about helping seniors and their families access quality care. The role combines community outreach, business development, networking, and marketing to drive referral growth and increase awareness of our services. This position offers an exciting opportunity to make a meaningful difference in people's lives while utilizing your sales, negotiation, and organizational skills. Responsibilities Develop and maintain strong relationships with referral sources including senior living communities, healthcare providers, hospitals, rehabilitation centers, and community organizations to promote home care services. Conduct routine in-person visits to referral partners and community organizations Conduct engaging product demonstrations and presentations to educate clients, partners, families, and healthcare professionals about available home care options and solutions, agency services and benefits Manage account portfolios by tracking client needs, follow-ups, and service satisfaction to foster long-term partnerships. Maintain accurate records of marketing activities, contacts, and referral sources Represent Home Sweet Home Care of SWFL at networking events, health fairs, community functions, and professional meetings Provide exceptional customer service by addressing inquiries promptly, resolving concerns efficiently, and maintaining open lines of communication. Collaborate with internal teams to tailor care plans that meet individual patient requirements while aligning with medical protocols. Analyze client feedback and market trends to identify opportunities for service improvement and growth strategies. Requirements High school diploma or equivalent required Degree in Marketing, Business, Healthcare Administration, or related field preferred Previous experience in healthcare marketing, home care, home health, hospice, senior living, sales, or community relations preferred Excellent communication and relationship-building skills Self-motivated with the ability to work independently Reliable transportation and valid drivers license required Proven experience in sales, particularly B2B (business-to-business) sales or medical sales, with a focus on healthcare or home care preferred. Strong negotiation skills combined with excellent communication abilities to effectively engage diverse stakeholders. Ability to deliver compelling product demos and technical presentations confidently. Exceptional organizational skills to manage multiple accounts and coordinate complex care arrangements efficiently. Proficiency in Microsoft Office suite including Word, PowerPoint, along with analysis skills for data interpretation. Outside sales experience is highly desirable, along with a customer-focused mindset committed to delivering outstanding service.
