Compassionate Connections Home Care is seeking a dedicated and compassionate Caregiver/HHA to join our team in Putnam County, FL. As a Caregiver/HHA, you will play a vital role in providing high-quality, personalized care to our clients in the comfort of their own homes. Role:
As a Caregiver/HHA, you will be responsible for providing routine care to our clients, including: Assisting with activities of daily living , such as bathing, dressing, and grooming Managing medication regimens and monitoring vital signs Preparing meals and performing light housekeeping tasks Providing emotional support and companionship to our clients
Responsibilities:
Provide individualized care to clients in accordance with their personalized care plans Develop and maintain positive relationships with clients, their families, and other healthcare professionals Maintain accurate and detailed records of client care and services provided Collaborate with other team members to ensure seamless care transitions and coordination Participate in ongoing training and professional development to stay current with industry best practices
Requirements:
High school diploma or equivalent required Current certification as a Home Health Aide (HHA) or willingness to obtain within a reasonable timeframe 0 years of experience in healthcare or a related field (no experience necessary, training provided) Ability to work independently and as part of a team Strong communication and interpersonal skills Valid driver's license and reliable transportation (may be required for travel to client visits)
