Every day, we serve approximately 1,500 patients living with advanced illness, wherever they call home, be it a private residence, a long-term care or assisted living facility, a hospital, or in one of our nine inpatient care centers. And most importantly, as the only non-profit hospice provider in the region, we never turn anyone away due to an inability to pay. The Hospice Aide provides compassionate, hands-on personal care to terminally ill patients in various settings. The aide supports patient comfort, dignity, and safety while adhering to the established plan of care. The role involves direct care tasks, assisting with mobility, and supporting family caregivers, with flexibility to work weekends, holidays, and across different care sites as needed.
Position Details Days/Hours:
Monday - Friday, 8:00am-5:00pm
Compensation:
Base salary $16.00-$17.50/hr; paid driving mileage, plus full benefits package including 23 days annual
PTO Position Location:
Home health covering Palatka and surrounding areas (Usually West Putnam County, including areas such as Interlachen, Keystone Heights, Melrose, etc...)
Responsibilities:
Provide personal care services in accordance with the plan of care and hospice regulations. Assist with patient ambulation and exercise routines. Perform basic procedures as directed by licensed clinicians. Observe and report changes in the patient's condition to a licensed nurse. Complete timely and accurate documentation per policy. Educate family or caregiver on proper patient care techniques. Maintain confidentiality of all patient information. Participate in interdisciplinary team meetings and other required sessions. Complete a minimum of 12 hours of job-specific training annually. Perform other duties as assigned
Education & Experience:
Current licensure, Certified Nursing Assistant (CNA) state of Florida. High school diploma or GED equivalent minimum. Six (6) months combined experience in home care, hospital setting, and/or long term care; experience in caring for terminally ill patients preferred. Current CPR/BLS certification. Valid Florida driver's license with acceptable driving record.
