Position Title:
Direct Care Worker Monday-Sunday 12hr. Shifts 36-40hrs./wk.
Summary:
12-hour shifts We are seeking a compassionate and energetic Direct Care Worker to join our dedicated team in providing exceptional support and care to individuals with diverse needs. In this vital role, you will assist residents with daily activities, promote independence, and ensure a safe, respectful environment. Your positive attitude and proactive approach will help enhance the quality of life for those you serve, fostering a warm and welcoming community. This paid position offers an opportunity to make a meaningful difference while developing valuable skills in caregiving, health support, and social work.
Responsibilities:
  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary
  • Provide personalized care plans tailored to each individuals needs, including medication administration and vital signs monitoring
  • Support residents with memory care needs, dementia care, Alzheimers care, and behavioral management strategies to promote comfort and stability
  • Help with meal preparation, feeding assistance, light cleaning, and maintaining a clean living environment in accordance with health standards
  • Document care activities accurately using Electronic Medical Record (EMR) systems while adhering to HIPAA regulations for patient confidentiality
  • Drive residents to appointments or outings safely while following all safety protocols and traffic laws
  • Collaborate with healthcare professionals, social workers, case managers, and family members to ensure comprehensive resident support
Requirements:
  • Prior experience in assisted living facilities, nursing homes, home care, or working with individuals with disabilities is preferred
  • Knowledge of medical terminology, medication administration, vital signs measurement, and basic first aid certification is essential
  • Ability to perform heavy lifting and physical tasks safely; experience with equipment such as Hoyer lifts or transfer devices is advantageous
  • Detailed conflict management skills and the ability to handle challenging behaviors with patience and professionalism
  • Valid driver's license and acceptable driving record for transportation duties; willingness to travel as needed
  • Effective communication skills to effectively interact with residents, families, and team members
  • Commitment to maintaining confidentiality in accordance with HIPAA standards and respecting residents dignity at all times Experience working with individuals with disabilities or behavioral needs is preferred CNA or HHA certifications are preferred Join us in creating a caring environment where every individual receives the support they need to thrive.
