Job Description

Job Summary M,W,Th 10a-2p Join our compassionate team as a Patient Companion, where your dedication and caring nature will make a meaningful difference in the lives of individuals requiring support with daily activities and medical needs. In this vital role, you will provide personalized assistance, emotional support, and companionship to patients across various care settings, including home care environments within the community. Your efforts will promote comfort, safety, and independence for residents and patients with diverse needs such as dementia, Alzheimer's, developmental disabilities, or recovery from illness. This paid position offers an opportunity to positively impact lives while working in a dynamic and rewarding environment. Duties Assist residents and patients with Activities of Daily Living (ADLs), including bathing and grooming reminders. Provide companionship to reduce feelings of loneliness and promote mental well-being through engaging conversations and social activities. Support medication administration following proper protocols and assist with vital signs monitoring to ensure patient safety. Aid in meal preparation and feeding for individuals with specific dietary or nutritional needs while maintaining hygiene standards through regular cleaning duties. Document care activities accurately using Electronic Medical Record (EMR) systems, update care plans, and communicate effectively with healthcare teams regarding patient status. Assist with light housekeeping tasks to maintain a clean and safe environment for residents or patients. Support behavioral management strategies for individuals with dementia or autism, implementing personalized care plans and conflict resolution techniques as needed. Experience Prior experience working in assisted living facilities, nursing homes, or home care environments is highly preferred. Demonstrated knowledge of caring for individuals with dementia, Alzheimer's disease, developmental disabilities, or autism spectrum disorders. Familiarity with medical terminology, HIPAA regulations, medication administration procedures, and basic first aid certification is advantageous. Experience operating EMR systems and performing vital signs assessments is desirable. Ability to lift heavy objects safely and perform physical tasks required for patient transfers or mobility assistance. Strong communication skills combined with compassionate caregiving approach; experience in social work or case management is a plus. Valid driver's license and clean driving record are preferred for roles involving transportation responsibilities.
