Job Overview RaphaCare is seeking compassionate, dependable, and professional Caregivers to provide exceptional non-medical home care services to seniors, older adults, individuals with disabilities, and clients recovering from illness, injury, surgery, or hospitalization. As a Caregiver, you will play a vital role in helping clients maintain their independence, dignity, safety, and quality of life while remaining in the comfort of their own homes. This position requires individuals who are patient, empathetic, and committed to delivering person-centered care. You will provide companionship, personal care, mobility assistance, and support with daily living activities while building meaningful relationships with clients and their families. Every interaction should reflect RaphaCare's commitment to compassionate, reliable, and respectful care. Responsibilities Provide personal care assistance, including bathing, grooming, dressing, toileting, oral hygiene, and incontinence care. Assist clients with safe transfers, ambulation, mobility, repositioning, and fall prevention. Prepare nutritious meals and assist with feeding when necessary while following dietary restrictions. Provide companionship, conversation, emotional support, and social engagement to reduce loneliness and improve overall well-being. Assist with medication reminders (non-medical), appointment reminders, and daily routines. Perform light housekeeping duties including laundry, changing bed linens, dishwashing, vacuuming, organizing, and maintaining a clean, safe living environment. Accompany clients to medical appointments, grocery shopping, errands, recreational outings, and community activities when authorized. Observe and promptly report changes in a client's physical, emotional, or cognitive condition to the Care Coordinator or Supervisor. Support clients living with Alzheimer's disease, dementia, Parkinson's disease, stroke recovery, or other age-related conditions using compassionate, person-centered approaches. Assist with post-hospital and rehabilitation-to-home transitions by encouraging independence while ensuring client safety. Accurately document care provided, daily activities, observations, and incident reports through the RaphaCare platform. Maintain client confidentiality and comply with HIPAA, agency policies, and all applicable state regulations. Qualifications & Skills Previous experience as a caregiver, home health aide, personal care assistant, CNA, or in a related healthcare role preferred. Experience caring for seniors, older adults, or individuals with disabilities is highly desirable. Excellent interpersonal, communication, and relationship-building skills. Compassionate, patient, dependable, and trustworthy with a strong desire to serve others. Ability to safely assist clients with mobility, transfers, and activities of daily living. Knowledge of infection prevention, safety practices, and emergency response procedures. Current CPR and First Aid certification preferred (or willingness to obtain). Reliable transportation, valid driver's license, and auto insurance (if transportation is required for the position). Ability to pass a background check and meet all state caregiver requirements. Comfortable using a smartphone or mobile app for scheduling, documentation, and communication. Ability to work independently while collaborating effectively with the RaphaCare care team.
