Job Description

Job Overview Join our compassionate team as a Personal Care Assistant / Home Health Aide (HHA), where you will play a vital role in enhancing the quality of life for individuals requiring assistance with daily activities. This energetic and rewarding position offers the opportunity to provide personalized care in various settings, including private homes, assisted living facilities, nursing homes, and hospice environments. Your dedication will help clients maintain independence, dignity, and comfort while receiving the highest standard of care. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts and transfer belts. Monitor patient health by observing vital signs, managing medication administration, and reporting changes in condition promptly. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques. Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations. Support clients with medical procedures such as catheter care, hospice care, and assisting with medical documentation as needed. Prepare nutritious meals and assist with light housekeeping tasks including cleaning and laundry to maintain a safe and comfortable environment. Engage in patient observation to ensure safety and well-being while fostering a positive relationship built on trust and respect. Requirements Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Strong knowledge of medical terminology, medication administration protocols, and patient care standards. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as HIPAA training or hospice care are a plus. Ability to work effectively with individuals with disabilities or developmental disabilities across diverse settings. Familiarity with EMR/EHR systems for accurate documentation of patient information. Excellent communication skills to collaborate with healthcare teams and family members effectively. Valid driver's license and reliable transportation are often necessary for home visits; flexibility to work various shifts including live-in arrangements is advantageous.
