Overview Join our compassionate team as a Home Health Aide/CNA and make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide essential support to patients in their homes or assisted living settings, promoting independence, safety, and comfort. Your dedication will help enhance quality of life for those with diverse needs, including seniors, individuals with disabilities, and those receiving hospice or memory care. This position offers a rewarding opportunity to apply your caregiving skills in a dynamic and supportive environment. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Observe and report patient conditions promptly, including changes in mental status or physical health, ensuring timely communication with healthcare teams Support patients with specialized care needs such as Alzheimer's disease, dementia, hospice care, and memory care strategies Document all patient interactions thoroughly Assist with household tasks including cleaning, laundry, meal preparation, and ensuring a safe environment to promote overall well-being Qualifications Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA), with current CPR certification preferred Experience working in nursing homes, assisted living facilities, or home environments is highly desirable Strong knowledge of medical terminology and procedures related to patient care and safety standards Demonstrated ability to perform patient observation accurately and respond effectively to changing conditions Compassionate caregiver with excellent communication skills and a genuine desire to support individuals with disabilities or chronic conditions Ability to work independently in home or community settings We offer competitive hourly rates with opportunities for specialized training.
