This vital role offers the opportunity to provide personalized, respectful, and attentive care within clients' homes, fostering independence and enhancing quality of life. We are committed to empowering our caregivers with the tools and environment to succeed while ensuring every client receives exceptional care tailored to their unique needs. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately Prepare nutritious meals and assist with feeding as needed, ensuring dietary restrictions are followed Administer medications safely according to care plans and document all medication activities in compliance with HIPAA regulations Support clients with mobility using equipment such as Hoyer lifts or transfer belts, ensuring safety during transfers Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Develop and follow personalized care plans tailored to each client's specific health conditions, including dementia, Alzheimer's, or developmental disabilities Provide emotional support and companionship to promote mental well-being and social engagement Collaborate with healthcare professionals, family members, and social workers to ensure comprehensive care delivery Adhere strictly to all safety protocols including CPR and first aid procedures; participate in ongoing training for emergency response Qualifications Proven experience in home & community care or assisted living environments; group home or nursing home experience is highly valued Knowledge of patient observation techniques, medication administration, and vital signs monitoring Familiarity with specialized care such as Alzheimer's/dementia care, memory care, hospice support, or working with individuals with autism or developmental disabilities Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Strong understanding of HIPAA regulations to maintain patient confidentiality at all times Valid CPR and First Aid certification; ongoing training preferred to stay current on safety standards Excellent communication skills with a compassionate approach towards clients and their families Ability to work independently while managing multiple responsibilities effectively Join us in providing dedicated home care that transforms lives!
