In Home Caregiver

Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs and observing for any changes in condition or behavior Administer medications accurately following care plans and HIPAA privacy regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support clients with memory care needs such as Alzheimer's or dementia by implementing specialized techniques and routines Use equipment like Hoyer lifts or resident's to ensure safe transfers and mobility assistance Maintain detailed documentation of patient care activities, observations, and any incidents in compliance with healthcare standards Perform light housekeeping duties such as cleaning client living areas to promote a safe and hygienic environment Collaborate with healthcare professionals to develop and update personalized care plans Provide emotional support and companionship to foster a positive, engaging atmosphere Skills Proven experience in home & community care, assisted living, nursing homes, or long-term care settings Knowledge of patient observation techniques, vital signs monitoring, and medication administration Strong understanding of dementia, Alzheimer's, autism spectrum disorder, developmental disabilities, hospice care, and memory care practices Ability to perform heavy lifting safely using appropriate equipment like Hoyer lifts or other assistive devices Excellent communication skills for working with clients, families, and healthcare teams Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information CPR and First Aid certification to respond effectively during emergencies Experience working with individuals with disabilities or special needs in group home or residential settings is preferred Ability to follow detailed care plans accurately while maintaining flexibility for individual needs Join us in delivering compassionate home health services that truly change lives.
