Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or other assistive devices. Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to promote overall health. Administer medications following prescribed care plans while maintaining compliance with HIPAA regulations to ensure confidentiality. Support clients with mobility challenges by providing safe transfer assistance and utilizing proper techniques for heavy lifting or transfers. Maintain a clean, safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces. Document care activities meticulously, update care plans regularly, and communicate effectively with family members and healthcare teams about patient progress or concerns. Skills Proven experience in home & community care settings, assisted living facilities, nursing homes, or group homes. Strong knowledge of patient observation techniques, medication administration, and vital signs monitoring. Familiarity with specialized care such as Alzheimer's/dementia care, memory care, hospice support, and working with individuals with developmental disabilities or autism. Certified in CPR and First Aid; additional certifications in HIPAA compliance preferred. Ability to perform physical tasks including heavy lifting and working with mobility aids like Hoyer lifts or resident's . Excellent communication skills for working collaboratively with clients' families, healthcare providers, and team members. Compassionate attitude combined with patience and adaptability to meet diverse client needs while ensuring safety at all times.
