This vital role involves providing compassionate, personalized assistance to promote independence, safety, and well-being within clients' homes or community settings. You will be a trusted partner in delivering high-quality care, ensuring comfort, safety, and dignity for those you serve. Whether assisting with mobility, medication management, or meal preparation, your efforts will create a positive impact every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to support their independence and comfort Monitor patients' health by observing vital signs, recognizing changes in condition, and reporting findings accurately while adhering to HIPAA privacy standards Administer medications as prescribed and maintain detailed care plans to ensure proper medication management Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Use specialized equipment such as Hoyer lifts or wheelchairs to safely transfer clients while practicing proper body mechanics and heavy lifting techniques Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing to promote hygiene and safety standards Document all care activities thoroughly in compliance with healthcare regulations and agency policies Collaborate with healthcare professionals, family members, and support teams to coordinate comprehensive care strategies for each individual Skills Proven experience in home & community care or assisted living environments with a strong understanding of patient observation and monitoring techniques Knowledge of dementia, Alzheimer's disease, memory care, autism spectrum disorder, developmental disabilities, hospice care, and working with individuals with disabilities or special needs Certifications in CPR and First Aid are required; additional certifications such as HIPAA compliance are highly valued Ability to perform heavy lifting safely using assistive devices like Hoyer lifts or resident lifts when necessary Excellent communication skills for working effectively with clients, families, and healthcare teams Strong organizational skills for managing multiple care plans and documentation requirements Compassionate demeanor combined with patience and adaptability to meet diverse client needs Join us in providing exceptional home care that empowers individuals to live their best lives!
