In this vital role, you will provide personalized care and support to individuals across various settings, including home health, assisted living, nursing homes, and group homes. Duties Assist individuals with Activities of Daily Living (ADLs) such as bathing, dressing, grooming. Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare teams Medication Reminder. Prepare nutritious meals and assist with meal planning in accordance with dietary needs and preferences Support patients with cognitive impairments such as Alzheimer's disease or dementia through memory care techniques and compassionate engagement Maintain a clean and safe environment by performing light housekeeping duties including cleaning patient areas and common spaces Document care activities thoroughly in compliance with healthcare standards and assist in updating individualized care plans regularly Skills Proven experience in home & community care, assisted living, nursing homes or group homes is highly preferred Strong understanding of HIPAA regulations to ensure patient privacy and confidentiality Excellent communication skills for working effectively with patients, families, and healthcare teams Certifications such as CPR, First Aid, training are mandatory; training provided if needed Join us today to become part of a compassionate team dedicated to providing exceptional care!
