Job Description
Overview We are seeking compassionate and dedicated PCA/HHA-Caregivers to provide exceptional in-home and community-based care to individuals with diverse needs. The ideal candidate will possess a strong commitment to patient well-being, with experience in senior care, home health, and working with individuals with disabilities or developmental challenges. This role offers an opportunity to make a meaningful difference in clients' lives by delivering personalized support that promotes independence, safety, and comfort. All positions are paid, including internships, ensuring a professional and rewarding work environment. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote dignity and independence. Monitor patient observations, including vital signs, medication reminders, and overall health status, documenting changes accurately. Support clients with mobility using equipment such as Hoyer lifts and assist with transfers safely. Provide companionship and emotional support, particularly for individuals with dementia or Alzheimer's disease, ensuring a safe and engaging environment. Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks including cleaning, laundry, and meal preparation. Provide medication reminders following prescribed protocols while maintaining compliance with HIPAA regulations to ensure confidentiality. Collaborate closely with healthcare teams to deliver comprehensive home & community care that meets each client's unique needs. Skills Strong knowledge of medical terminology, hospice care practices, catheter care, and dementia/memory care techniques. Experience working in assisted living facilities, nursing homes, or long-term care environments is highly desirable. Ability to observe patient conditions carefully and respond promptly to changes in health status or behavior. Excellent communication skills for effective interaction with clients, families, and healthcare professionals. Familiarity with medical procedures such as vital signs measurement and CPR certification is required; additional certifications like hospice care or dementia training are a plus. Demonstrated caregiving skills in home health settings or working with individuals with disabilities across diverse environments.
