Responsibilities Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs, medication administration, and overall condition updates Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support patients with mobility using equipment such as Hoyer lifts and ensure safe transfer procedures Maintain accurate care plans and document patient progress while adhering to HIPAA regulations for confidentiality Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment Offer companionship and emotional support to promote mental well-being and social engagement Collaborate with healthcare professionals on patient care strategies, including hospice or dementia care plans Experience Candidates should possess prior experience in home & community care settings, assisted living facilities, nursing homes, or group homes. Experience working with individuals with developmental disabilities, autism spectrum disorder, Alzheimer's disease, or dementia is highly valued. Familiarity with patient observation techniques, vital signs monitoring, medication administration, and emergency response procedures such as CPR and first aid is essential. Prior work involving resident's (personal needs), working with disabilities or in long-term care environments will strengthen your application. Knowledge of HIPAA compliance and experience supporting patients through complex care plans are important. Candidates should demonstrate strong communication skills, patience, adaptability, and the ability to handle heavy lifting safely when required.
