Job Description
Overview Join our compassionate team as an In-Home Care Provider and make a meaningful difference in the lives of individuals requiring personalized care and support. This vital role involves delivering high-quality, patient-centered assistance within clients' homes and community settings. As an In-Home Care Provider, you will promote independence, safety, and well-being by providing tailored care that meets each person's unique needs. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, and respiration to ensure health stability Administer medications accurately following care plans while maintaining HIPAA compliance for patient privacy Support patients with meal preparation and nutritional needs, ensuring dietary restrictions are followed Utilize specialized equipment such as Hoyer lifts or wheelchairs to safely transfer residents or clients with mobility challenges Conduct patient observation to identify changes in condition or behavior, reporting findings to healthcare professionals promptly Maintain a clean and safe environment by performing light housekeeping tasks including cleaning and sanitation of living spaces Follow detailed care plans designed by healthcare teams to deliver consistent and personalized support Assist with medical procedures like medication administration or wound care as trained and authorized Provide emotional support and companionship to reduce feelings of isolation for individuals with dementia, autism, or developmental disabilities Skills Strong knowledge of home health practices, hospice care, dementia care, Alzheimer's care, and memory care techniques Experience working in assisted living facilities, nursing homes, group homes or long-term care environments Proficiency in patient monitoring, vital signs measurement, and recognizing signs of health deterioration Excellent communication skills for collaborating with healthcare teams and engaging with clients compassionately Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information Demonstrated ability to follow detailed care plans accurately and document services provided effectively Prior experience working with individuals with disabilities or developmental challenges such as autism or other cognitive impairments Knowledge of CPR and first aid procedures to respond swiftly during emergencies Capable of working independently while managing multiple clients in a home or community setting Join us in delivering exceptional home care that empowers individuals to live healthier, happier lives.
