Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, noting vital signs and changes in condition to ensure safety and well-being Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Administer medications accurately following care plans while maintaining strict adherence to HIPAA regulations for patient privacy Support clients with mobility using equipment such as Hoyer lifts or transfer belts, ensuring proper safety protocols are followed Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Document care activities meticulously, updating care plans regularly and communicating effectively with healthcare teams or family members Requirements Proven experience in home & community care, assisted living, nursing homes, or group homes is highly preferred Knowledge of patient observation techniques, vital signs monitoring, medication administration, and emergency procedures including CPR and first aid Experience working with individuals with developmental disabilities, autism, Alzheimer's disease, dementia, or memory care needs Ability to lift heavy objects safely and operate mobility aids like Hoyer lifts or wheelchairs when necessary Strong understanding of HIPAA regulations to protect patient confidentiality at all times Excellent communication skills to collaborate with healthcare professionals, families, and clients effectively Valid certifications in CPR and first aid; additional training in hospice or long-term care is a plus Join us in delivering compassionate home health care that truly transforms lives.
