Duties Assist residents or clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and dignity. Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals. Support medication administration following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary, ensuring safety during meal times. Maintain a clean and safe environment by performing light cleaning duties, laundry, and organizing personal spaces. Use specialized equipment such as Hoyer lifts or resident's to assist with mobility and prevent injury during transfers. Document care activities meticulously, including patient observations, progress notes, and adherence to care plans for seamless communication within the team. Qualifications Prior experience working in assisted living facilities, nursing homes, or home & community care settings is highly preferred. Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), and working with individuals with developmental disabilities or autism. Certifications such as CPR, First Aid, and HIPAA compliance are required; additional training in hospice care or heavy lifting is a plus. Ability to work effectively with diverse populations including seniors and individuals with disabilities or chronic conditions like dementia or Alzheimer's disease. Strong communication skills for collaborating with healthcare teams, residents' families, and other caregivers. Physical stamina for tasks involving patient monitoring, assisting with mobility, and performing heavy lifting when necessary.
