This role offers an energizing opportunity to provide personalized care in home and community settings, promoting independence, safety, and well-being. Whether assisting with personal care, medication management, or engaging in social activities, you'll be a vital part of creating a warm, supportive environment for those you serve. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately in compliance with HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with feeding as required Support medication administration following care plans and safety protocols to ensure proper timing and dosage Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Provide emotional support and companionship to promote mental well-being, especially for individuals with dementia or developmental disabilities Collaborate with healthcare professionals to implement care plans effectively and report any concerns or incidents promptly Requirements Prior experience in home & community care, assisted living, nursing homes, or group homes is highly preferred Certification in First Aid and CPR is required; additional training in Alzheimer's care, dementia care, hospice care or working with individuals with autism is a plus Knowledge of HIPAA regulations to ensure patient privacy and confidentiality Ability to lift heavy objects safely and perform physical tasks such as transferring residents using assistive devices like Hoyer lifts Strong observational skills for patient monitoring and recognizing changes in health status Excellent communication skills to interact effectively with clients, families, and healthcare teams Valid driver's license and reliable transportation are often necessary for community-based caregiving roles Embark on a rewarding journey where your caregiving skills foster independence and improve quality of life.
