Job Description

Job Summary We are seeking compassionate and dedicated In Home Caregivers to provide personalized care and support to clients in their own homes. The ideal candidate will have experience working in diverse healthcare settings, including Level II trauma centers, hospital medicine, emergency medicine, and post-acute care environments. This role offers an opportunity to make a meaningful difference in clients' lives by delivering high-quality, patient-centered care across various medical disciplines such as geriatrics, medical-surgical, outpatient, and sub-acute care. The position requires a flexible, empathetic individual committed to maintaining the dignity and independence of those they serve. Responsibilities Provide comprehensive in-home care tailored to individual client needs, including assistance with daily activities, medication management, and mobility support. Monitor clients' health status and report any changes or concerns to healthcare providers promptly. Assist with personal hygiene, grooming, and nutrition to promote overall well-being. Support clients with mobility and transfers using proper techniques to ensure safety. Maintain accurate documentation of care provided and communicate effectively with family members and healthcare teams. Follow all safety protocols and infection control procedures to ensure a safe environment for clients and staff. Collaborate with multidisciplinary teams when necessary, especially for clients with complex medical conditions such as those requiring PICU or ICU-level care. Requirements Proven experience in healthcare settings such as Level I Trauma Centers, Geriatrics, Medical-Surgical units, PICU, OR experience, or Post-Acute Care environments is highly desirable. Experience working in hospital medicine, emergency medicine, outpatient clinics, or teaching hospitals is a plus. Strong understanding of patient care protocols across inpatient and outpatient settings. Ability to work independently while managing multiple client needs effectively. Excellent communication skills and compassionate bedside manner. Valid driver's license preferred for travel between client locations. Must pass background checks and adhere to all safety and infection control standards. This role is vital in supporting the health and independence of individuals within their homes while providing caregivers with a rewarding opportunity to apply their clinical skills in a community setting.
