Job Description

Job Summary Join our compassionate team as an In Home Caregiver, where your dedication and skills will make a meaningful difference in the lives of individuals requiring support in their daily routines. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs and observing changes in condition; document findings accurately in compliance with HIPAA regulations Administer medications as prescribed and maintain detailed medication administration records Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Utilize specialized equipment such as Hoyer lifts or other assistive devices to ensure safe transfers and mobility support Support clients with dementia, Alzheimer's, or other memory-related conditions through patient-centered care strategies Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning and laundry Develop and follow personalized care plans in collaboration with healthcare professionals and family members Provide emotional support and companionship to promote mental well-being and social engagement Ensure compliance with all safety protocols including CPR and First Aid certifications; respond promptly to emergencies or changes in condition Experience Prior experience working in home health, assisted living, nursing homes, or group homes is highly preferred Demonstrated ability to work effectively with individuals with developmental disabilities, autism, or dementia Experience providing hospice or palliative care is a plus Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times Proven ability to perform patient observation and monitoring for signs of distress or deterioration Hands-on experience with patient care procedures such as vital signs measurement and medication administration Ability to lift heavy objects safely using assistive devices like Hoyer lifts; physically capable of supporting residents' mobility needs Familiarity with creating care plans aligned with healthcare providers' instructions Strong interpersonal skills for working compassionately with clients, families, and healthcare teams Join us in delivering heartfelt caregiving that transforms lives!
