Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe health status to identify any changes or concerns promptly Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and confidentiality Prepare nutritious meals aligned with dietary restrictions and preferences, ensuring proper meal hygiene and safety standards Operate assistive devices such as Hoyer lifts and perform heavy lifting when necessary to support mobility and transfers Maintain detailed documentation of patient care, progress notes, and incident reports in compliance with healthcare standards Support clients with cognitive impairments such as Alzheimer's disease or dementia through memory care techniques and compassionate engagement Conduct patient observation to detect signs of distress or deterioration, escalating concerns to healthcare professionals as needed Perform light cleaning duties to maintain a safe and hygienic environment in accordance with home & community care standards Collaborate with healthcare teams to develop and implement personalized care plans tailored to each individual's needs Assist with transportation or accompany clients on outings within the community, promoting social interaction and independence Experience Prior experience working in assisted living facilities, nursing homes, group homes, or home health environments is highly preferred Demonstrated ability to work effectively with individuals with developmental disabilities, autism spectrum disorder, or memory impairments such as Alzheimer's or dementia Proven skills in patient monitoring, medication administration, and vital signs measurement Experience providing hospice or long-term care services is advantageous Knowledge of HIPAA regulations ensuring confidentiality and privacy of patient information Hands-on experience using mobility aids like Hoyer lifts or other assistive devices for safe patient transfers Strong understanding of care plans development and execution in various healthcare settings Ability to handle physically demanding tasks including heavy lifting while maintaining safety standards for both caregiver and client Join us in delivering heartfelt care that truly makes a difference!
