Overview 3 days per week 4 hours per day. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patient health by checking vital signs, observing changes in condition, and reporting findings accurately while adhering to HIPAA privacy regulations. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support medication administration following care plans and safety protocols to ensure proper medication management. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Use specialized equipment like Hoyer lifts or resident's to assist with mobility and prevent injury during transfers. Document care activities meticulously in compliance with healthcare standards and assist in updating personalized care plans regularly. Skills Proven experience in home health, assisted living, nursing home environments, or group homes with a focus on senior or disability care. Knowledge of patient observation techniques, vital signs monitoring, and dementia or Alzheimer's care strategies. Certification in CPR and First Aid is required; additional certifications such as HIPAA compliance or hospice care are a plus. Ability to work effectively with individuals with developmental disabilities or autism spectrum disorder through direct support techniques. Strong communication skills for collaborating with healthcare teams, residents' families, and social workers to ensure comprehensive care. Physical stamina for heavy lifting tasks and assisting residents using equipment like Hoyer lifts or resident's . Compassionate attitude combined with excellent organizational skills to follow detailed care plans and medication schedules.
