Overview Join our compassionate team as an In-Home Caregiver, where your dedication will make a meaningful difference in the lives of individuals requiring personalized care and support. This role offers an opportunity to provide essential assistance in clients' homes while promoting independence, safety, and overall well-being. At Percipience Home Care, we are a tech-enabled home care agency focused on delivering high-quality, client-centered services. In addition to traditional caregiving, this role includes conducting Smart Step Assessments to help prevent in-home falls and supporting telehealth visits to enhance care coordination. Key Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor client condition, including observing changes and reporting concerns Prepare meals and assist with basic nutrition and hydration needs Support medication reminders in accordance with care plans Provide companionship and support for clients with dementia, Alzheimer's, or other conditions Maintain a clean and safe home environment through light housekeeping Smart Step Assessments Conduct in-home Smart Step Assessments to identify fall risks Evaluate home safety (trip hazards, lighting, mobility barriers) Document findings and provide recommendations to improve safety Complete and submit assessment documentation accurately Telehealth Support Assist clients with telehealth visits , including setting up devices and ensuring smooth communication with providers Help clients stay engaged during virtual appointments Communicate relevant updates to the care team as needed Qualifications Previous caregiving experience (home care, assisted living, nursing home, or similar) preferred Experience working with seniors or individuals with disabilities CPR and First Aid certification preferred Strong communication and interpersonal skills Ability to safely assist with mobility and physical support tasks Comfortable using mobile apps and technology for scheduling, documentation, and telehealth support Understanding of client confidentiality and professionalism Why Join Percipience Home Care Flexible scheduling Opportunity to work in a growing, tech-enabled home care agency Meaningful work supporting clients in their homes Ability to expand your skills with assessments and telehealth support Join Percipience Home Care and be part of a team committed to delivering compassionate, high-quality care that helps clients live safely and independently at home.
