Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patient health by taking vital signs, observing changes in condition, and reporting findings to healthcare professionals promptly. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary to promote proper nutrition and hydration. Administer medications following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality and privacy. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning personal spaces, laundry, and common areas. Support clients with memory care needs related to dementia or Alzheimer's disease through patience, engagement, and structured routines. Document care activities accurately in compliance with healthcare standards and assist in updating personalized care plans regularly. Skills Proven experience in home & community care or assisted living environments with a focus on patient observation and monitoring. Knowledge of first aid, CPR certification, and emergency response procedures to ensure safety at all times. Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, or dementia-related conditions such as Alzheimer's or memory care needs. Ability to perform heavy lifting safely using equipment like Hoyer lifts or resident transfer devices. Strong communication skills for effective collaboration with healthcare teams, residents' families, and other caregivers. Experience with medication administration, vital sign measurement, and implementing care plans aligned with nursing or hospice protocols. Flexibility to adapt to different client needs while maintaining professionalism and compassion in all interactions.
