Job Description

Overview Join our compassionate home care team and make a meaningful difference in the lives of individuals requiring personalized support and assistance. As a Home Care professional, you will provide essential caregiving services that promote independence, safety, and well-being in clients' homes. This role offers an exciting opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. Your dedication will help clients maintain their dignity and quality of life while fostering a safe and supportive environment. Responsibilities Observe and monitor patients' health status, including vital signs and overall condition, to ensure safety and well-being. Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support. Administer medications accurately following medical documentation protocols while adhering to HIPAA regulations for patient privacy. Support clients with specialized care needs including Alzheimer's care, dementia care, hospice support, catheter care, and memory care. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities precisely and maintain medical records. Utilize assistive devices like Hoyer lifts safely during patient transfers to prevent injury. Prepare nutritious meals and assist with light cleaning, laundry, and household chores to promote a clean and healthy environment. Collaborate with healthcare professionals by documenting medical information accurately and communicating any changes in patient condition promptly. Provide compassionate companionship and emotional support to foster a positive home environment for clients. Follow all safety protocols including CPR certification requirements to ensure preparedness for emergencies. Qualifications Prior experience in home health, assisted living, nursing homes, or long-term care settings is highly desirable. Knowledge of medical terminology, medication administration, catheter care, hospice care, dementia care, and Alzheimer's support is preferred. Demonstrated ability to work with individuals with developmental disabilities or other special needs. Strong patient observation skills combined with the ability to assist with ADLs effectively. Familiarity with EMR/EHR systems for accurate documentation of medical records and care activities. Valid CPR certification; additional certifications such as HIPAA training are advantageous. Experience working with patients requiring memory or dementia care is a plus. Ability to handle physical tasks including lifting using Hoyer lifts and providing mobility assistance safely. Excellent communication skills coupled with a caring attitude to build trust and rapport with clients. $40.00 - $50.00 per hour
