DutiesProvide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility supportMonitor patients health by taking vital signs, observing changes, and reporting findings accurately while adhering to HIPAA privacy standardsAssist with medication administration and manage care plans tailored to each individual's needsPrepare nutritious meals and assist with meal planning in accordance with dietary requirementsSupport patients with mobility using equipment such as Hoyer lifts or other assistive devices, ensuring safety during transfers and movementMaintain a clean and safe environment by performing light housekeeping tasks like cleaning patient areas and laundryDocument patient progress, update care records, and communicate effectively with healthcare teams or family membersSkillsProven experience in home health, assisted living, nursing homes, or group home settings is highly valuedKnowledge of patient observation techniques, dementia care (including Alzheimer's), memory care, and hospice supportCertification in CPR and First Aid; experience with HIPAA compliance is essential for maintaining confidentialityAbility to work with individuals with developmental disabilities, autism spectrum disorder, or other special needsStrong skills in assisting with ADLs, medication management, vital signs monitoring, and patient care documentationPhysical capability for heavy lifting and assisting with transfers using equipment like the Hoyer lift; stamina for long shifts requiredExcellent communication skills for collaborating with clients families, healthcare providers, and team membersJoin us today to become a vital part of a caring community dedicated to enhancing lives through compassionate home care!
