Care Coach | Paid.care
Location:
North Vernon, Indiana (and surrounding areas within a 2-hour radius) Position Overview At Paid.care, our mission is to ensure that families receive the professional support and guidance they need while caring for their loved ones. As a Care Coach, you serve as a vital resource in empowering family caregivers through education, advocacy, and hands-on oversight. This role bridges the gap between clinical requirements and compassionate home care. You will be responsible for caregiver onboarding, conducting in-home orientations, and performing comprehensive assessments to ensure every client receives the highest standard of care in a safe environment.
Core Responsibilities Onboarding & Orientation:
Conduct professional in-home orientations for new families, managing all necessary documentation and program enrollment.
Clinical & Safety Assessments:
Evaluate client care needs and assess the home environment for safety, suitability, and compliance.
Caregiver Development:
Review caregiver competencies, provide targeted training on ADLs/IADLs, and ensure caregivers feel confident in their daily tasks.
Case Oversight:
Perform periodic supervisory visits and ongoing check-ins to monitor the effectiveness of the care plan.
Interdisciplinary Collaboration:
Partner with Care Managers and families to maintain care plan integrity and participate in team meetings to optimize client outcomes.
Operational Growth:
Provide feedback on clinical processes and support the scaling of operations as Paid.care expands its footprint.
Compensation & Logistics Rate:
$65-$100 per visit (determined by visit type and case complexity).
Travel:
Reimbursed mileage and approved travel expenses.
Schedule:
Flexible, autonomous scheduling based on client needs and personal availability.
Requirements:
Must possess a reliable, insured vehicle and a valid driver's license.
Candidate Qualifications Experience:
1+ years of experience supporting seniors, adults with disabilities, or family caregivers.
Clinical Knowledge:
Strong familiarity with
ADL/IADL
assessments and care planning.
Instructional Readiness:
Must be willing to become a certified CPR Instructor to help train our network of caregivers.
Field Expertise:
Background in home care, case management, or healthcare navigation is highly preferred.
Technical Proficiency:
Comfortable utilizing digital documentation systems and case management software.
Communication:
Exceptional interpersonal skills with the ability to train and mentor caregivers with patience and clarity.
