Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring dignity and respect at all times Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to support accurate patient assessments Support medication administration under supervision, adhering to HIPAA regulations and care plans Provide compassionate Alzheimer's and dementia care, implementing memory care techniques to promote comfort and safety Conduct patient observation and monitoring, noting any changes in condition or behavior and reporting to healthcare staff promptly Assist with mobility using equipment such as Hoyer lifts, ensuring safe transfer and positioning Prepare nutritious meals and assist with meal service, following dietary restrictions and care plans Maintain a clean and safe environment by performing routine cleaning, sanitation, and infection control procedures Support hospice, home health, and community care services by assisting with home visits and providing social support Collaborate with interdisciplinary teams, including social workers and nursing staff, to develop and implement personalized care plans Document all care activities accurately, maintaining confidentiality and compliance with HIPAA standards Engage with residents and their families, providing emotional support and fostering a positive environment Participate in ongoing training, including CPR and first aid certification, to stay current with best practices in patient care and safety Qualifications Previous experience in group homes, assisted living, nursing homes, or home & community care is highly preferred Knowledge of patient observation, vital signs, medication administration, and care planning Experience working with people with developmental disabilities, autism, Alzheimer's, dementia, or memory care needs Ability to perform light lifting and assist with mobility and transfers using appropriate equipment Strong understanding of HIPAA regulations and patient confidentiality standards Certification in CPR and first aid required; additional certifications in hospice, home health, or social work are a plus Excellent communication skills and a compassionate, energetic attitude Ability to work independently and as part of a team in a fast-paced environment Flexibility to work various shifts, including evenings, weekends, and holidays as needed Physical stamina and resilience to support residents' needs and perform routine cleaning and caregiving tasks Embark on a rewarding career where your caring nature and energetic spirit can truly make a difference.
