Direct Service Worker (DSW)

Job Summary Essential Functions:

To provide in-home assistance with activities of daily living and supervision or assistance with in-home and/or community activities/tasks for consumers as per their individualized Plan of Care. Services will be provided at the time and place assigned. Maintains confidentiality of any and all confidential information at all times. Assists consumer with personal hygiene. Provides assistance with activities of daily living including but not limited to the following: eating; bathing; dressing; grooming; transferring; ambulation and/or turning and positioning consumer; toileting needs including assistance with bedpans, bedside toilets and urinals Performs light housekeeping tasks including vacuuming, dusting, mopping floors, cleaning bathroom and kitchen and making the bed. These tasks should only be performed in the area of the home used exclusively by the consumer, or those parts of common areas used solely by the consumer. Direct Service Worker should make sure that pathways are free from obstructions. Prepares meals from consumer's supplies adhering to special diet, preplanned menus, or planning meals. Safely stores food after cooking. Goes shopping at nearby market to purchase necessary food and household supplies with funds supplied by consumer. Leaving receipt and change with consumer. Washing, sorting, folding and proper storage of consumer laundry and bed linens. This does not include laundry or bed linens for individuals other than consumer living in the same home. Provides medication oversight - assistance with self-administration of prescription and non-prescription medication. Unless otherwise noted on Registered Nurse Medication Delegation form, this assistance is limited to the following: verbal reminders; assistance with opening the bottle or bubble pack; reading the directions from the label; checking the dosage according to the label directions; and/or assistance with ordering medication from the drug store or pharmacy. Provides assistance with scheduling medical appointments; Accompanying the consumer to medical appointments when necessary and arranging for transportation to and from appointments. Supervising the consumer to ensure safety at home and/or in the community. Mental and/or emotional support or stimulation for consumer as needed. Completing required documentation daily, legibly, and only using ink. Shows dependability by being punctual (on-time); maintains consistent attendance Maintaining a neat and attractive appearance. Maintaining a safe environment for consumers. Following safe lifting practicing at all times. Reporting changes in consumer's status or requests for changes in services to Supervisor immediately upon discovery Other duties and or tasks assigned by your immediate supervisor and/or included in the consumer's individual plan of care.
