The agency, through its programs & services, encourages the inclusion of people with disabilities into the mainstream of society, including social, recreational, family & work activities. A community based Senior Skills Trainer for our Personal Care Attendant Program. The Skills Trainer will educate consumers on how to successfully manage their consumer-directed hands-on care. The Senior Skills Trainer will also be primarily responsible for other skills training department wide tasks & processes. This Personal Care Management Skills Trainer will cover Cape Cod & Martha Vineyards Islands & Nantucket area in Massachusetts including over the bridge. Bilingual English preferred in any languages Travel required, cover a 60-mile radius of your home address Must have a valid driver\'s license Must have reliable vehicle to travel throughout assigned service area Mileage reimbursement 2-day orientation in Stoughton in person Training in Stoughton for 2-3 weeks. Essential Job Functions Follow-up on assigned functional skills training in a timely manner as determined by contract. Assess consumers &/or surrogate ability to manage PCA services. Train consumers &/or surrogates the skills needed to manage their PCA program as outlined in the Mass Health Service Agreement & according to the Personal Care Management (PCM) Contract. Maintain confidentiality in all consumer related issues. Attend in-service, supervision & staff development meetings when requested by Supervisor or Manager. Demonstrate a working knowledge of program policies & procedures & Mass Health regulations. Demonstrate a commitment to the Independent Living philosophy of consumer control. Must report all suspected incidents of consumer sexual/physical abuse & neglect to the Disabled Person Protection Commission (DPPC). Work within company policies & in accordance with the PCM Contract in order to complete & track for receipt of required documentation within required timelines. Assigned referrals & intake documentation Service Agreements & Assessments for re-evaluations Supportive documentation for intakes & re-evaluations as needed Return all phone calls from surrogates & consumers within 72 business hours Work within policies to complete & submit internal requirements: Progress Notes Dayforce, Outlook Schedule Mileage & Expense Forms Return all required email communications within 48 business hours. Follow up with Supervisor &/or Program Assistant on requests for skills training & tracking reports: Non-billing, overbilling & ineligible consumers Provide training & ongoing support for EVV Major problems requiring skills training Consumer status updates Intake, Quarterly & Annual Reviews Perform other duties as assigned by the Skills Trainer Supervisor Qualifications Experience providing services for people with disabilities & knowledge of PCA programs is preferred. Experience or education in teaching or counseling is helpful. Knowledge of community resources & social service systems is beneficial. Excellent communication, organizational, & writing skills. Basic computer skills in Microsoft Office/Outlook & the ability to navigate databases. Specialized training provided as needed. Required Education Bachelor?s Degree preferred; GED or High School Diploma required.
