Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor and document patient health status, including vital signs and behavioral changes, while adhering to HIPAA privacy regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support medication administration and ensure proper care plans are followed accurately Utilize specialized equipment such as Hoyer lifts or resident's personal assistive devices to ensure safe transfers and mobility Conduct patient observation to identify potential health concerns or safety risks promptly Maintain a clean and safe environment through light housekeeping tasks like cleaning, laundry, and organizing living spaces Provide emotional support and companionship to promote mental well-being, especially for individuals with dementia or memory care needs Collaborate with healthcare professionals, family members, and social workers to deliver comprehensive home & community care Requirements Proven experience in home health, assisted living, nursing homes, or group home settings with a focus on senior care or disabilities Knowledge of patient observation techniques, medication administration, and vital signs monitoring Certifications in CPR and First Aid are required; additional certifications such as HIPAA training are preferred Experience working with individuals with developmental disabilities, autism, Alzheimer's disease, dementia, or hospice care is highly valued Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary Strong communication skills for effective collaboration with clients' families and healthcare teams Compassionate attitude combined with a proactive approach to safety and patient well-being Join us in creating a supportive environment where every individual receives personalized care that fosters independence and dignity.
