Monitored and documented vital signs, weights, intake and output, and changes in resident condition. Provided compassionate, resident-centered care while promoting dignity, comfort, and independence. Assisted residents with ambulation, transfers, positioning, and range-of-motion exercises using proper body mechanics and lifting equipment. Responded promptly to call lights and resident requests to ensure safety and well-being. Observed and reported physical, emotional, and behavioral changes to licensed nursing staff.
