In this vital role, you will provide comprehensive support to clients in their homes, ensuring their safety, comfort, and well-being. Your energetic and proactive approach will help promote independence and enhance quality of life for those you serve. This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those recovering from illness or injury. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by checking vital signs, observing for changes in condition, and reporting findings accurately while maintaining HIPAA compliance Prepare nutritious meals tailored to individual dietary needs and assist with medication administration following care plans Provide companionship and emotional support to reduce feelings of loneliness or isolation for clients with dementia, Alzheimer's, or developmental disabilities Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe and hygienic environment Support clients during outings or community activities to promote social engagement and independence Follow detailed care plans created by healthcare professionals to ensure consistent and personalized care delivery Experience Prior experience in home health care, assisted living, nursing homes, or group homes is highly desirable Proven ability to work with individuals with developmental disabilities, autism spectrum disorder, dementia, or memory care needs Knowledge of first aid, CPR certification, and medication administration procedures Experience working with residents who have complex medical conditions such as hospice or palliative care needs Familiarity with patient observation techniques and monitoring vital signs in accordance with healthcare protocols Demonstrated ability to handle heavy lifting safely and operate mobility aids like Hoyer lifts or wheelchairs Background in social work or long-term care settings that emphasizes compassionate patient support Join us in delivering exceptional home care that empowers individuals to live safely and comfortably in their own environments.
