We are seeking compassionate, reliable and motivated individuals to join our team as Community Living Support Life Skills (CLS) Coach/Mentors. As a CLS Mentor, you'll work in groups with individuals with disabilities, helping foster positive community involvement, planning personalized goals, and helping them develop greater independence. Whether you are supporting someone during a community outing, teaching daily living skills, or simply being a positive role model, you'll have the opportunity to make a real difference in someone's life! No prior experience is necessary. We provide paid training and ongoing support. Summary of essential job functions Build positive relationships with individuals and their families. Support individuals in developing life skills that increase independence and improve quality of life Assist individuals in working toward goals identified through Person-Centered Planning Encourage community involvement and participation in meaningful activities Follow supervision guidelines established by CMH or Network 180 Implement therapy, behavior, and dietary plans as directed by agency professionals Safely transport individuals to and from community activities using your personal vehicle Maintain accurate documentation through our online systems and required reports Assist with personal care needs, including feeding, dressing, and restroom assistance when needed Serve as a positive role model, advocate, and mentor Perform other duties as assigned Minimum requirements Be able to lift up to 50 lbs Valid Driver's license, acceptable driving record, and proof of car insurance. Must be able to pass a background check Must demonstrate a passionate interest for helping people with disabilities Must be mature, reliable, and honest Must demonstrate strong communication and relationship building skills and be able to work well with little supervision Must have a flexible schedule
