In this vital role, you will deliver personalized care directly in clients' homes, promoting independence and enhancing quality of life. Responsibilities Assist clients with daily activities such as bathing, dressing, grooming, and mobility support Provide companionship and emotional support to foster a positive environment Monitor clients' health status and report any changes or concerns promptly Support medication reminders and light housekeeping tasks to maintain a safe living space Follow care plans meticulously to ensure all client needs are met with compassion and professionalism Communicate effectively with family members and healthcare professionals as needed Maintain detailed documentation of care provided and any incidents or observations Experience Previous experience in home health or personal caregiving preferred but not required Strong understanding of elderly care, Geriatrics, and post-acute care principles Experience with individuals requiring assistance due to chronic conditions or disabilities Ability to adapt care approaches based on individual client needs and preferences Compassionate attitude with excellent communication skills Reliable, punctual, and committed to providing consistent support in a home setting Join us at Silver Lining Serenity Care LLC where your dedication transforms lives!
