In this vital role, you will provide personalized care and support to adults with varying needs, including those with developmental disabilities and mental disabilities. Your energetic approach will help create a safe, nurturing environment where residents feel valued and empowered. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring long-term care, while working within a supportive team committed to excellence in home and community care. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor residents' vital signs and observe for any changes in health or behavior, documenting findings accurately and promptly Prepare nutritious meals tailored to individual dietary needs, ensuring safety and adherence to care plans Administer medications following prescribed schedules while maintaining compliance with HIPAA regulations and medication protocols Maintain a clean and safe environment through regular cleaning, laundry, and sanitation tasks in accordance with health standards Support residents during activities, social outings, and community engagement to promote mental stimulation and social well-being Follow detailed care plans designed by healthcare professionals to ensure personalized support for each resident's unique needs Requirements Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care settings is highly preferred Valid certifications such as First Aid, CPR, and HIPAA compliance are required; additional training in Alzheimer's & dementia care is a plus Demonstrated ability to work with individuals with developmental disabilities or autism spectrum disorders with patience and understanding Strong observational skills for patient monitoring and recognizing signs of distress or health deterioration Ability to perform heavy lifting safely when assisting residents with mobility or transfers using equipment like Hoyer lifts or resident's personal devices Excellent communication skills for collaborating with healthcare teams, residents, families, and social workers Commitment to maintaining confidentiality and adhering to all safety protocols in home health or hospice care environments Join us in providing compassionate caregiving that transforms lives!
