POSITION SUMMARY
The Care Center Hospice Aide/CNA is a member of the interdisciplinary team who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The Care Center Hospice Aide is responsible for observing the patient, reporting these observations, and then documenting observations and care performed in compliance with the medical plan of care. The Hospice Aide/CNA is committed to the philosophy, goals and objectives of Angela Hospice and the Felician Franciscan Core Values.
POSITION REPORTS TO
DIRECTOR
OF INPATIENT SERVICES ESSENTIAL JOB FUNCTIONS
Performs personal care needs, as established in the plan of care, after receiving patient assignments and attends to his/her requests promptly Provides patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality Meets safety needs of patient while using equipment safely and properly (foot stools, side rails, O2 etc.) Provides personal care including baths, back rubs, oral hygiene, and changing bed linen, in accordance with the patient's plan of care, as often as assigned Assists in dressing and undressing patients as assigned Assists in feeding the patient as assigned Takes and records oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered Provides proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence Assesses and reports on patient's condition and significant changes to the registered nurse, including interpersonal issues between the patient and caregiver/family Assists in ambulation and exercise as instructed by the registered nurse or therapist Provides normal range of motion and transfers/positions as directed Offers and assists with bedpans and urinals Provides assistance as assigned with light laundry needs Keeps patient's living area clean and orderly, as assigned Adheres to the organization's documentation and care procedures and standards of personal and professional conduct Respectful of patient and family/caregiver environment and patient's personal needs The above statements are only meant to be a representative summary of the major duties and responsibilities performed by the occupant of this job. OTHER DUTIES
Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services Ensures compliance with the Medicare conditions of participation and other state regulations that govern the provision of healthcare Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies Participates in organizational monitoring of the quality of medical services and quality improvement initiatives Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes Fulfills the obligation of requested and/or accepted assignments Supports department operations and special projects that may be assigned Performs tasks which are supportive in nature to the essential functions of the position, which may be altered based upon the need of the organization Meets on-going requirements to remain compliant with Federal/State or other regulatory guidelines Participates in efforts and initiatives to promote growth and longevity of the organization within your departmental scope Performs other duties as directed by the President/CEO, Director of Inpatient Services or other assigned designee

SUPERVISORY RESPONSIBILITY
This position does not have any supervisory responsibilities, but is required to orient employees as needed and will act as a mentor to staff and volunteers.
PHYSICAL REQUIREMENTS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The position requires sitting and standing for long periods. The position is also exposed to infectious diseases and requires lifting of a minimum of 50+ pounds regularly. It also requires bending, stooping, turning and twisting on a regular basis. He/she requires the ability to operate a computer, phone and other office equipment as needed. Manual dexterity is also required to perform tasks including computer tasks.
WORK ENVIRONMENT
This position operates in a health care setting. The role requires walking to various areas of the building and working directly with patients and other team members.
EXPECTED HOURS OF WORK
This is a position in a hospice care facility open 24 hours a day, 365 days a year. Scheduled regular, additional and holiday shifts as well as work during inclement weather will be required. TRAVEL This position does not include travel routinely, but may require travel as necessary.
QUALIFICATIONS
Current CNA certification in the State of Michigan At least 18 years of age Ability to read, follow written instructions, and document care given Understands hospice philosophy and is comfortable providing specialized care to the terminally ill Is self-directing with the ability to work with little direct supervision
