PT Home Care University Home Care Inc - 2.0 Livonia, MI Job Details Part-time $70 - $90 an hour 14 hours ago Qualifications Physical Therapy License Full Job Description University Home Care Inc is seeking a dedicated and compassionate Physical Therapist (PT) to join our home care team in the Southeast Michigan Region. Must be willing to travel. As a PT Home Care provider, you will deliver high-quality, personalized rehabilitation services to patients in the comfort of their own homes, helping them regain mobility, improve function, and enhance their quality of life. Responsibilities Conduct comprehensive patient assessments and develop individualized treatment plans. Provide therapeutic interventions and monitor patient progress. Educate patients and their families on treatment plans and home exercises. Document all patient interactions and maintain accurate medical records. Collaborate with interdisciplinary care teams to coordinate patient care. Ensure compliance with all regulatory and company policies. Requirements Valid Licensed Physical Therapist credential in the state of Michigan. Experience in home health care or outpatient physical therapy preferred. Strong communication and interpersonal skills. Ability to work independently and manage time effectively. Reliable transportation to travel to patient homes within the Southeast Michigan Region.
