Job Description

Job Overview We are seeking a compassionate and dedicated Direct Care Worker to provide exceptional support and assistance to individuals with diverse needs. In this vital role, you will be a key part of enhancing residents' quality of life by delivering personalized care in various settings such as assisted living facilities, nursing homes, group homes, and home care environments. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor vital signs such as blood pressure, pulse, and temperature to ensure residents' health and well-being. Administer medications accurately following prescribed schedules and document all medication activities within Electronic Medical Record (EMR) systems. Provide specialized care for individuals with dementia or Alzheimer's disease, implementing memory care techniques and behavior management strategies. Support residents with assisted living needs by preparing nutritious meals, helping with personal hygiene, and maintaining a clean and safe environment through light cleaning duties. Engage residents in social activities to promote mental stimulation and emotional well-being while fostering independence. Collaborate with healthcare professionals, social workers, and family members to develop personalized care plans that reflect each resident's unique needs. Drive residents when necessary for appointments or community outings, ensuring safety at all times. Experience Prior experience working in nursing homes, retirement homes, or assisted living facilities is highly preferred. Demonstrated knowledge of long-term care practices including home & community care settings. Experience assisting individuals with developmental disabilities or disabilities requiring specialized support is advantageous. Familiarity with dementia care techniques and managing behaviors associated with Alzheimer's disease is desirable. Proficiency in taking vital signs, medication administration, and using EMR systems to document care activities accurately. Hands-on experience operating mobility aids such as Hoyer lifts or other assistive devices is a plus. Strong caregiving background complemented by excellent communication skills to effectively support residents' physical and emotional needs.
