Job Description

Central MN Senior Care has been dedicated to providing compassionate assisted living services since 1993. With a focus on fostering meaningful relationships and enhancing quality of life, the organization operates 5 sites across multiple communities, serving clients in residential settings. We are seeking a dedicated and compassionate Direct Care Provider to join our team. This vital role involves supporting residents with daily activities, ensuring their safety, and promoting independence within a respectful and caring environment. The ideal candidate will possess a genuine desire to make a positive impact on the lives of seniors and individuals with disabilities through attentive, personalized care. Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor residents' vital signs and health status, reporting any concerns to healthcare professionals promptly. Administer medications accurately following prescribed protocols and maintain detailed documentation using electronic medical record (EMR) systems. Provide specialized care for residents with dementia or Alzheimer's disease, implementing memory care strategies and behavior management techniques. Support residents in social activities and community engagement to promote mental and emotional well-being. Maintain a clean, safe, and organized environment by assisting with light housekeeping tasks such as cleaning resident areas and common spaces. Operate equipment to assist with transfers and mobility needs. Prepare nutritious meals in accordance with dietary guidelines and individual preferences, ensuring proper food safety standards are met. Drive residents when necessary for appointments or outings, adhering to safety protocols and traffic laws. Document care activities thoroughly in EMR systems, ensuring compliance with regulatory standards. Prior experience working in assisted living facilities, nursing homes, or home & community care environments is highly desirable. Knowledge of long-term care practices including medication administration, vital signs monitoring, and dementia or Alzheimer's care. Experience assisting individuals with disabilities or developmental disabilities is preferred. Familiarity with caregiving techniques such as personal care support, behavior management, and use of mobility aids. Strong interpersonal skills with the ability to communicate effectively with residents, families, and team members. Ability to operate EMR systems efficiently while maintaining confidentiality of resident information. Key Responsibilities:
Household & Environment Assist with light housekeeping, laundry, and maintaining a clean, safe living environment. Report maintenance or safety concerns promptly. Participate in staff meetings and training sessions. Safety & Compliance Follow all facility policies, procedures, and state regulations. Support infection control practices and emergency preparedness procedures. Maintain resident dignity, privacy, and confidentiality at all times.
Qualifications:
High school diploma or equivalent preferred. Previous caregiving experience in assisted living, home care, or long-term care preferred but not required. Compassionate, patient, and dependable. Strong communication and teamwork skills. Ability to work flexible shifts, including evenings, weekends, and holidays as needed.
Physical Requirements:
Ability to lift, push, and pull up to 50 lbs. Ability to stand, walk, and move frequently throughout the shift.
