Individualized Home Supports Specialist - IHS Home Supports

Certified Home Care is hiring for Independent Home Support workers (IHS) to work 1:1 with clients in their home and in the community. The position locations are in Champlin and Blaine ; Candidates must be comfortable driving personal vehicle between locations. Must pass a background check, have a valid driver's license, a reliable vehicle , proof of insurance on that vehicle, and ability to transport clients. Essential Functions Duties would include but not limited to: Evaluating the home situation to determine what health teaching will be required, what assistance from family will be provided, and to determine if other services are needed Coordination and connection to community services Provide skill training in an environment where the skill will be used. Support development of decision-making skills and informed choices in all aspects of daily living, including selection of service providers, goals and methods, location and decor of residence, roommates, daily routines, leisure activities and personal possessions. Periodic individual program support reviews Ability to teach, coach and mentor client on job/school activities, Health management, Household management, organization, and stability Interpersonal skills and communication Memory compensation strategies Problems solving assistance, and Time Management Safety and self-defense Transportation resources Increase consumers' independence by teaching life skills and activities such as; Meal planning, grocery shopping, house cleaning duties, cooking, organizational skills, financial management and self-care Coaching and working with the clients on emergency situations, completing all necessary paperwork and working with the clients to live in their homes as independently as possible

Required Qualifications:

High school diploma Excellent driving record, reliable car , and current auto insurance

Preferred Qualifications:
