This position includes hands-on assistance with personal hygiene, grooming, toileting/showering, and bathroom-related care . Applicants must be comfortable providing respectful and professional personal care support to the client. 15hrs/weekly

Overview:
Join our team as an Individualized Home Supports (IHS) Worker with Nurture Home Care, providing 15 hours per week of support to help a client maintain independence and well-being. Responsibilities include assisting with behavioral needs, light housekeeping, laundry, and helping with medication reminders. This paid role offers a meaningful opportunity to provide personalized care in a supportive environment. Duties 1. Daily Living Support Assist individuals with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and meal preparation 2. Community Integration Support participation in community activities, appointments, shopping, and recreational outings Encourage social interaction and skill development Provide transportation as needed 3. Skill Development & Independence Teach and reinforce independent living skills Support budgeting, grocery planning, and household management Implement person-centered goals according to Individual Support Plans (ISP) 4. Behavioral & Emotional Support Provide positive behavioral support strategies De-escalate challenging behaviors using approved techniques Promote dignity, respect, and self-advocacy 5. Documentation & Compliance Complete daily progress notes and required documentation accurately and on time Follow agency policies, HIPAA guidelines, and state regulations Report incidents, health concerns, or changes in condition promptly 6. Collaboration Communicate effectively with case managers, guardians, and interdisciplinary team members Attend team meetings and training as required Support medication reminders and monitor health-related needs Maintain a clean, safe, and organized living environment Experience Experience with different personalities and backgrounds CPR and First Aid Certified - We provide free training! 18 years or older High School Diploma or equivalent Current Driver's License, auto insurance, and a clean record Willing to take clientele to and from appointments Willingness to help client read and understand documents Pass a MN DHS Background Study Ensure all documentation is completed in a timely manner. Provide person-centered support that promotes independence, responsible decision-making, and overall well-being. Serve as a positive role model and mentor, offering guidance and encouragement in daily life situations. Support individuals in developing coping skills and navigating life challenges through creative problem-solving and goal setting. Provide prevention-focused support and therapeutic interventions as appropriate within the scope of the role. Establish and maintain open, supportive communication with individuals experiencing mental or emotional challenges. Complete accurate and timely documentation in accordance with agency and DHS requirements.
