Community-based and in-home support About the Position Minnesota Home Health Services LLC is hiring a Direct Support Professional / In-Home Supports Worker to provide person-centered support to an individual receiving 245D services. This position focuses on helping the person served build independence, participate in the community, maintain health and safety, and work toward goals listed in their service plan. Strong documentation skills are required. Staff must be able to complete timely, accurate, and professional service notes in accordance with Minnesota 245D requirements and company policy. Main Responsibilities Provide respectful, person-centered support Assist with daily living skills, independence, and community participation Support the person with problem-solving, reminders, and skill-building Assist with navigating public transportation, including Metro bus or other approved transportation Provide approved community support, including occasional driving Complete accurate service documentation and EVV/timekeeping Follow the person's service plan, company policies, and 245D requirements Report concerns, incidents, safety issues, or service barriers to a supervisor Maintain professional boundaries and confidentiality Transportation This is primarily a public transportation support position. The person served is independent with scheduling and typically uses Metro bus or other approved transportation options. Driving may be required approximately 25% of the time for approved community support needs. Applicants must have a valid driver's license, proof of insurance, and reliable transportation if driving is required.
