In this vital role, you will provide essential support to clients in their homes or community settings, promoting independence, safety, and well-being. Your proactive approach and empathetic nature will help create a comfortable environment for those you serve, ensuring they receive the highest quality of care with dignity and respect. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts. Monitor and record vital signs like blood pressure, pulse, temperature, and respiration to track health status accurately. Administer medications and perform catheter care following medical instructions and maintaining strict adherence to HIPAA regulations. Support clients with memory care needs, dementia, Alzheimer's disease, and other cognitive impairments through specialized techniques and compassionate engagement. Document all patient interactions and medical information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure seamless communication among healthcare providers. Provide companionship and emotional support while assisting with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment. Collaborate with healthcare teams in hospice or long-term care settings to deliver holistic patient care tailored to individual needs. Skills Proven experience in home health care, assisted living, nursing homes, or long-term care environments. Strong knowledge of medical terminology, medication administration protocols, and vital signs monitoring. Skilled in working with individuals with developmental disabilities, dementia, Alzheimer's disease, or other cognitive impairments. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Ability to perform patient observation diligently and respond promptly to changing health conditions. Competence in assisting with complex procedures such as catheter care or hospice-related tasks. CPR certification and understanding of safety protocols for patient handling using equipment like Hoyer lifts or transfer devices. Excellent communication skills to effectively collaborate with clients' families and healthcare teams while maintaining confidentiality under HIPAA guidelines.
