Job Description

Core Responsibilities Bathing & Hygiene :
Bathe residents as scheduled (bed bath, shower, tub, or whirlpool) or as needed, following care plans and doctor's orders. Maintain privacy and dignity throughout.
Personal Care :
Assist with grooming (shaving, hair care, nail care, oral hygiene, denture care, skin care, cosmetics) and dressing/undressing as needed.
Skin & Health Monitoring :
Check skin condition, report any changes or concerns to nursing staff. Assistance with
Mobility :
Help residents transfer between beds, chairs, wheelchairs, and tubs; position them safely.
Linen & Room Maintenance :
Change bed linen according to facility standards, keep rooms clean and clutter-free.
Mealtime Assistance :
Pass trays, check diets, assist with food preparation, offer fluids, and record intakes.
Elimination Care :
Assist with toileting, bedpans, urinals, and other elimination needs.
Activity Support :
Help residents to and from dining rooms, beauty shops, and activities.
Call Response :
Answer resident calls promptly and address needs.
Documentation :
Accurately chart care provided and report any incidents or changes in condition. Team & Safety Duties Work collaboratively with other staff to ensure excellent resident care. Follow infection control, abuse prevention, and HIPAA protocols. Maintain a clean, safe environment and report hazards or incidents immediately. Comply with facility policies and licensing regulations. Physical Requirements Able to lift up to 50 lbs. (or assist with larger residents). Unlimited bending, stooping, and pivoting. Good vision, hearing, and communication skills.
