Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to ensure comfort and dignity. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate documentation in accordance with HIPAA regulations. Support medication administration and help implement individualized care plans under supervision. Prepare nutritious meals tailored to dietary needs while maintaining cleanliness and safety standards within the living environment. Operate equipment such as Hoyer lifts and assist with heavy lifting to ensure safe transfers and mobility assistance. Maintain a clean, safe, and welcoming environment by performing light housekeeping tasks including cleaning common areas and resident rooms. Provide emotional support and companionship, fostering a positive atmosphere that promotes social interaction and mental well-being. Skills Proven experience in home & community care, assisted living, nursing homes, or long-term care environments. Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or autism. Certification in CPR, First Aid, and HIPAA compliance; additional certifications such as hospice care or nursing are advantageous. Ability to accurately monitor vital signs and recognize signs of distress or health deterioration. Strong communication skills for collaborating with healthcare teams, residents, families, and social workers. Physical stamina for activities involving heavy lifting and assisting with mobility; familiarity with equipment like Hoyer lifts is preferred. Experience working within care plans and medication administration protocols to ensure safety and adherence to healthcare standards.
